ChatGPT Web sessions generally do not have a `refresh_token`; when a real `id_token` is missing, CPA, Cockpit, Codex, and AxonHub will construct placeholder JWT claims that Codex can parse. Codex and Codex-Manager will keep an empty `refresh_token`; AxonHub will write a placeholder value when a real `refresh_token` is missing.
